Athletic waiver & consent

This Athletic Participation Agreement, Release of Liability, and Consent ("Agreement") is entered into by the parent or legal guardian ("you") on behalf of the participating minor athlete ("Athlete") and AFC. You accept this Agreement when you sign it during booking.

1 · Assumption of risk

I understand that participation in soccer and multi-sport athletic training involves inherent risks, including physical contact, falls, and the risk of injury. I knowingly and voluntarily assume all such risks on behalf of the Athlete.

2 · Release of liability

To the fullest extent permitted by law, I release and hold harmless AFC, its owners, coaches, and staff from any claims, demands, or liabilities arising out of the Athlete's participation, except for injury caused by gross negligence or willful misconduct.

3 · Medical authorization

I confirm the Athlete is in good health and physically able to participate. In the event of injury or emergency where I cannot be reached, I authorize AFC to seek appropriate medical care for the Athlete.

4 · Media consent

I grant AFC permission to capture and use photos or video of the Athlete during sessions for coaching and promotional purposes. You may opt out at any time by contacting us.

5 · Parental consent

I certify that I am the parent or legal guardian of the Athlete, that I have authority to accept this Agreement on their behalf, and that the information I provide is accurate.
